"ELDERS" is a web-enabled medical eligibility determination and case management system integrated into the OKDHS systems environment. Because applications for Home and Community Based Services have both a medical and financial eligibility determination, ELDERS streamlines the process for OKDHS nurse and case workers by scoring applications based on the final determination and pushing it forward to services many days sooner than the previous paper process. With ELDERS Remote, nurses are able to work offline and later sync their case data. Utilizing electronic notifications to service delivery programs, ELDERS saves OKDHS several man hours of faxing and scanning while placing much needed services in the consumer's home many days sooner while creating an effective method for tracking case loads and application timeliness. ELDERS utilizes the CAPSTONE framework. Click here to learn more about CAPSTONE.
